I recommend a method of presenting with the structure points highlighted in the bracket sections:

“This is an Ultrasound Image of a 74 year old man’s gallbladder showing an eccentric 6mm lesion intimate with the wall which is thin. Visualised Liver is Normal in Appearance (Observation). In summary a gallbladder lesion which I suspect represents a small polyp (Interpretation and Diagnosis) — however a differential diagnosis is an impacted stone (Differential Diagnosis). I would want to check a decubitus view to ascertain if this is fixed or mobile (Management).”
